When Do You Submit a BSR-9 Form?
Checklist of Attachments to Include in the BSR-9 Form
The BSR-9 form is submitted after:
The public review period has ended for a BSR-8, and any comments received have been addressed by the Committee.
Approval of the appropriate supervisory board has been obtained.
1. To open a BSR-9 form, go to the Staff Tab.
2. Select a Committee Responsible.
3. Under ANSI choose the BSR-9 form.
4. Click on the Go button.
NOTE: Staff Secretaries are now able to attach eligible Record(s) to the BSR-9 form. Eligible Records are Records belonging to:
the Primary Committee Responsible (chosen on the Staff Tab)
all committees below the Primary Committee Responsible
the committee one level above the Primary Committee Responsible
5. The New BSR-9 form will open in a new window. To attach BSR-8 Record(s) to the BSR-9 form:
a. Select the associated BSR-8 from the scroll menu (A).
b. Click on the >> button to move it to the selected BSR-8 box (B).
c. Click on the Get BSR-8 Info button.
d. You will be taken to the New BSR-9 form. Based on the selected BSR-8, some of the fields are auto-populated into the BSR-9 form. The following describes the information auto-populated on the New BSR-9:
1) Hyperlink to the BSR-8 form (C): Opens the BSR-8 form in a new window.
2) Associated Record #s (D): Records currently associated with the BSR-9. This field is auto-populated by the selected BSR-8 form.
Only records in BSR-8 with record level of Board Approved will automatically be moved into the Associated Record #s field of the BSR-9.
Excluded Records # (E). All records will be moved by the system to the Excluded Records #s column when the following conditions are met:
a) All records that are NOT Board Approved.
b) All records that were originally excluded from the BSR-8s records (selected for BSR-9 only) EVEN if it is with a Board Approved.
The Record #s from the BSR-8 will display the BSR-8 number followed by the Record # to provide further clarity.
The record types that staff should place in the Excluded column on the BSR-8 will be automatically excluded on the BSR-9 even if staff forgot to exclude them from the BSR-8.You can exclude any additional records from a BSR-8 by selecting a record in the Associated Record #s column and using the >> button to move it to the Excluded Record #s from this BSR-8 column (E).
The << button will not be available once the BSR 8 is selected to the associated BSR-9.

This field is auto-populated based on the selected BSR-8 form.
This field can be updated. If updated, be sure to note the change in the Explanatory Notes field (below).

11) Scope Summary (O): Enter the scope summary.
12) Balance (N): If the consensus body associated with this submittal appears to lack balance as described in the ANSI Essential requirements (see 1.3, 2.3), please attach evidence or an explanation of outreach efforts undertaken to achieve balance. Evidence attached? Select Yes, No, or N/A.
13) Did a PINS deliberation take place in connection with this standard (See ANSI Essential Requirements 2.5)? (O) If yes, then attach a copy of the required PINS deliberation report(s): Select Yes, No, or N/A.
14) Total number of unresolved objections to the proposed action on the standard resulting from all public review period(s) (PUBLIC REVIEW ONLY). Attach Evidence of attempted resolution (if applicable) (P). Enter number, if applicable.
15) Date(s) on which unresolved objections (PUBLIC REVIEW AND CONSENSUS BODY), attempts at resolution and substantive changes, were provided to the consensus body for consideration (i.e. recirculation). Attach Evidence of this (if applicable) (Q)
Enter dates of all recirculation ballots with outstanding negatives.
16) Were all unresolved objector(s) (PUBLIC REVIEW AND CONSENSUS BODY) informed of their right to appeal to standards Developer? Attach Evidence of this (if applicable) (R): Select Yes, No or N/A.
17) Did any unresolved objectors (PUBLIC REVIEW AND CONSENSUS BODY) complete the appeals process available through the standards developer? If yes, attach documentation of appeal proceedings (S). Select Yes, No or N/A.
18) Date of Final Consensus Body Vote (i.e., date vote was closed or date of latest change of vote by consensus body member; if multiple records included in BSR-9, provide date for the record that has latest date of final vote) (mm/dd/yy) (T). Enter the date using the date format “mm/dd/yy”.
19) State the applicable numerical requirements for consensus as established in your ANSI-accredited procedures: At least two-thirds affirmative vote of the committee membership excluding any "not voting" and "disapproved without comment" responses. Click Yes to certify that these requirements have been satisfied (U): Select Yes or No.
20) Click YES if there are statements from patent holders (patent letters of assurance) that have been received regarding the proposed ANS. Please attach a copy of such statements (V): Select Yes or No.
21) BSR-9 Summary of Consensus Body Vote File (W): Auto-generate the report, including supporting documentation if applicable, then use the Browse button to attach the material. You can refer to the Checklist of Attachments to Include in the BSR-9 Form for more information.
When you click the Generate BSR-9 Consensus Body Vote Report link, an auto-generated tally report appears in another window. You can print or save the report and attach it to the BSR-9.
22) C&S Staff Comments (X): Staff can enter any comments they have here.
Example for C&S Staff Comments: Number of delegates on the committee; reason(s) for difference between CBVR tallies and actual ballot tallies; reasons why certain records are not currently included; reasons why certain records are included; anything to prevent ANSI Admin from questioning the reports and records included or excluded.
NOTE: The C&S Staff Comments field will automatically populate if there were staff comments on the BSR-8 form.
23) Explanatory Notes (Y): This field allows engineers to track changes to the BSR-9 form, and add an explanation for any revisions made to the BSR-9 form and/or ANSI Box.
The Explanatory Notes field is included in the automatic email that is sent when a change is made to the BSR-9 form.
Example for Explanatory Notes: ANSI Admin will provide reasons why submittals are on hold or rejected; changes the engineers have made; and any comments from ANSI.
24) Click on the Submit BSR-9 button.
The Consensus Body Vote Report underwent a number of substantial changes. There is no longer one roster per tally sheet. Instead, each tally sheet will have the members’ login names in the tally, with interest classifications on the left hand side. Then there will be one combined roster covering all members who voted on any of the items included in the BSR-9.
When the interest classification is not identified in Personify, the vote is not associated to any of the available interest categories. The report includes the interest classification breakdown and committee roster in one document. The report is generated from the BSR-9, which retrieves the tally from the ballots associated with the selected records.
Along with the Summary of Consensus Body Vote is the historical roster.
These are attached via the Browse button in section (W) above.
1. Attach evidence of attempted resolution (only for objections received during the Public Review period). Responses to ballot comments count as evidence of attempted resolution.
2. Attach evidence of notification to the consensus body which are the emails received when the ballots are opened and closed.
3. Attach evidence of notification of appeals process (to objectors of the Consensus Body and objectors during the Public Review period).
4. Attach evidence of statements from patent holders.
5. Attach all tally documentation.
